Our Federal grants require us by contract to verify every applicant’s income at the time of intake. This includes the income for everyone living in the household who is age eighteen (18) or older.
If you are missing specific eligibility documentation with your application, you will be asked to submit the documentation within 72 business hours. If you do not provide all of the essential documentation within the allotted timeframe, your application will be denied.

The form(s) below can be used by those who do not have pay stubs for the past four weeks to provide as proof of employment income; or have not been with a new job long enough to receive a paycheck.
Everyone on the application who reports they are age 18 or older will be asked if they have income to report. If the answer is no, the household member will complete a Self-Declaration of Zero Income as part of the application. This is only valid for 30 days.
Only use this Zero Income Recertification Form if your application is over 30 days old and our office has requested you Re-certify your zero income status.
