Park County Community Partnerships
Since 2017, our direct services have been coordinated with organizations and agencies that offer intensive case management services to low-income families in Park County, Wyoming. YCAN coordinates services with local case managers and social workers to provide low-income clients with a one-stop-shop of essential human services.
YCAN’s Park County Goals:
- Working with the Park County Court Supervised Treatment Program, coordinated services and financial assistance are provided to low-income participants so they can transition out of poverty.
- Working with Heritage Health Clinic, coordinated services, and financial assistance are provided to low-income patients to address immediate needs and improve health and well-being.
- Our various COVID Help funds are appropriated specifically “to prevent, prepare for, or respond to coronavirus.” Coordinated services and funds to assist any low-income individual or community are considered a legitimate “response to coronavirus,” given the funded activities are identified. Housing, Health, and Social/Behavioral Development services have been identified in Park County.
The Coronavirus required some adjustments, simplified procedures, & re-deploying emergency resources to address COVID-19 community needs. Park County Help has remained fully functional both in-person and remotely using online meetings, phone calls, text messaging, electronic applications, and email to conduct business with case managers.
We have also created a new Case Management Portal that is password protected to give our community partners access remotely to Park County and Hot Springs County Help.
We continuously pursue all options to effectively serve as many community partner clients in need with the amount of funding granted to each project in Park and Hot Springs Counties.
